Commentary: Death certificate reporting needs to be fixed

Something is wrong with the death reporting when there is a fivefold difference in the reported rates of fall mortality between Alabama and Vermont, two states in the United States. The problem is not unique here, as even the Swedish have trouble with their death certificate data.
